10. What supporting documentation must be submitted along with my application?

1. Proof of Age for the Applicant & Co-Applicant (include one of the following)

1. A copy of the applicant’s driver’s license or Non-Driver License;
2. A copy of the applicant’s birth certificate; or
3. A copy of the applicant’s passport or Passport Card

2. Proof of Ownership

1. A copy of the real property tax bill and/or receipt showing the address of the homestead and the name of the Eligible Taxpayer as being the one responsible for said taxes. Additional documents may be requested by the Collector’s Office for circumstances requiring further verification.
